Dermoscopic criteria may assist in the diagnosis of LM and LMM, but lack full sensitivity and specificity. While histologic diagnosis is the gold standard, atypical melanocytes are rare and sparsely distributed in these two conditions. LM is thus difficult to distinguish from melanocytic hyperplasia in photodamaged skin, and lesion margins are hard to define. Some authors have recently highlighted the importance of questioning any histopathological diagnosis of junctional nevus, dysplastic nevus, or atypical lentiginous nevus for lesions occurring in photodamaged skin on the face and neck. Nevi in this area are typically papuletype lesions of the same color as adjoining skin, but are clinically different from LM. Any of the above nevus diagnoses may underestimate an LM lesion as a benign junctional melanocytic proliferation. Immunohistochemistry can be of use in these equivocal cases. Typical markers include S100, human melanoma black-45 antigen (HMB-45), melan-A (MART-1), and microphthalmia-associated transcription factor (MITF), all of which are highly sensitive to melanocytes. However, these markers fail to differentiate melanocytic nevi from melanoma lesions; P16 protein expression and proliferation markers Ki-67 and pHH-3 help with this. Identification of specific mutations in the BRAF, NRAS and KIT genes may be of use, especially given the recent availability of targeted drugs. Reflectance confocal microscopy (RCM) was introduced to improve diagnostic accuracy, define the extent of a lesion,

Difficult-to-diagnose facial melanomas: Utility of reflectance confocal microscopy in uncovering the diagnosis

LM: Lentigo maligna pAK: pigmented actinic keratosis RCM: reflectance confocal microscopy M elanoma on sun-damaged skin, commonly known as lentigo maligna (LM, akin to melanoma in situ) and lentigo maligna melanoma (akin to invasive melanoma), is mostly found on the head and neck of elderly patients. This type of melanoma, particularly when found early, may be difficult to diagnose on clinical ...

Lentigo maligna treated with topical imiquimod: dermatoscopy usefulness in clinical monitoring.

Dermoscopy has its usefulness well established in the diagnostic evaluation of melanocytic lesions. Recently, however, it has also shown to be an important tool in monitoring therapeutic response to various dermatoses. We report the case of an elderly patient diagnosed with lentigo maligna of difficult surgical management, which we have chosen to treat with topical imiquimod. The dermoscopic mo...

Diagnosis and management of lentigo maligna: a review

Lentigo maligna is a melanocytic neoplasm occurring on sun-exposed skin, usually on the head and neck, of middle-aged and elderly patients. It is thought to represent the in situ phase of lentigo maligna melanoma. The ill-defined nature and potentially large size of lesions can pose significant diagnostic and treatment challenges. The goal of therapy is to cure the lesions in order to prevent d...

Lentigo maligna mimicking invasive melanoma in Mohs surgery: a case report

Lentigo maligna is a lentiginous proliferation of atypical melanocytes confined to the epidermis, typically on chronically sun-damaged skin. Following biopsy and exclusion of invasive disease, therapy may involve Mohs surgery, topical treatment or radiotherapy. However, lentigo maligna often involves adnexal structures, creating histological difficulty in distinguishing these foci from invasive...

Clinical usefulness of reflectance confocal microscopy in the management of facial lentigo maligna melanoma.

Facial lentigo maligna melanoma can be a diagnostic challenge in daily clinical practice as it has similar clinical and morphological features to other lesions such as solar lentigines and pigmented actinic keratoses. Confocal microscopy is a noninvasive technique that provides real-time images of the epidermis and superficial dermis with cellular-level resolution. We describe 3 cases of suspec...
